What to bring to your
first appointment

The Loudoun Community Health Center offers a sliding fee scale to patients who do not have private insurance and are within a certain income level. To help determine if you are eligible for the sliding fee scale, please bring with you to your first appointment:

• Proof of income (pay stub or letter stating you do not have an income)
• Photo ID

If you have private insurance, please bring your insurance card with you to every visit.
To help your practitioner better understand your health needs, please also bring all medication you are currently taking to your first medical visit.
